(f) Block 35. Fill in this block as follows. If additional space is needed, use a continu-
ation sheet. DA Form 2407-1.
-
Enter telephone number (including autovon number) and address of the unit
forwarding the claim to the NMP on the first line of this block.
-
Write "Replace Defective Part" in this block.
-  Clearly state all factors which contributed to the failure, such as the type of
operation, terrain, and climatic conditions.
-
Explanatory sketches and photographs should be used as needed.
-
Record date that the part failed.
-
Record contract number (DAAE07-81-C-6205)
-
Record vehicle inset-vice date found on DA Form 2408-9, Block 23, in the
logbook.
-
Record USA registration number of the truck.
-
Record month-day-hour (MDH) code and date of manufacture as found on the
Vehicle Identification Data Plate.
-
Record MlLSTRIP document number, and the address of the unit receiving
disposition instructions and/or replacement warranty part(s).
-  Overseas units must clearly record their DODAAC number. Failure to enter
this number will seriously delay overseas shipments.
2. PREPARATION OF DA FORM 2402.
a.  Use of DA Form 2402. The DA Form 2402 must be filled out and attached to any and all
defective part(s) removed from the vehicle. Refer to figure 2 for an example of the completed DA
Form 2402.
b. Completing DA Form 2402. Fill out the form as follows:
(1) Block 1. Enter the support agency (DODAAC or UIC) where the item is to be ex-
changed.
(2) Block 2. Enter the Julian date the item was prepared for exchange.
(3) Block 3. Enter the unit or organization (DODAAC or UIC) originating the exchange.
(4) Block 4. Place an "X" in the block to identify an exchange or ElR exhibit. When used
for a warranty claim identification, place a "W" in the ElR block.
(5) Block 5. Enter the national stock number of the item.
(6) Block 6. Enter the noun nomenclature of the item to be exchanged; e.g., starter,
carburetor, etc.
