(1) Section I. (To be filled in by originating organization.)
(a) Heading. Put a "VV" in the EIR space, and write in the organization's issue pri-
ority .
(b) Block 1a. Enter the name of organization submitting the request.
(c) Block 1b. Enter location of unit making the request. Overseas units must enter
their APO.
(d) Block 1c. Enter unit identification code of the originating organization.
(e) Block 2. Enter the AM General Corporation serial number found on the Vehicle
Identification Data Plate or the Serial Number Plate. The Identification Plate is located on the lower
left corner inside the left cab door. The Number Plate is located on the left side of the truck's frame
to the rear of the driver's seat.
(f) Block 3. Enter the name of the end item.
(g) Block 4. Leave blank.
(h) Block 5. Enter vehicle model designation for the defective part found on the
Vehicle Identification Data Plate.
(i) Block 6. Enter national stock number of the vehicle.
(j) Block 7. Enter symbol of the military maintenance activity doing the work.
(k) Block 8. Enter appropriate utilization code from Table 1.
(I) Block 9. If defective part(s) can be reported on DA Form 2406, Material Readi-
ness Report, place an "R" to the right of the "No" block.
(m) Block 10. Leave blank.
(n) Block 11. Enter mileage reading (rounded to the nearest mile) from the odometer
at the time of failure.
